INSURANCE PLANS

• We are in network with most plans offered by:

  • Aetna

  • Blue Cross/Blue Shield

  • Cigna

  • United HealthCare

  • UMR

  • We do not accept Medicare.
    

• Aetna: in networt.

• Blue Cross/Blue Shield, Regent, Anthem, Empire, and the Federal Employee Program: in network

• Blue Cross/Blue Shield PimaFocus plans: not in network.

• Cigna: in network

• Tricare: certain Tricare plans only.

• United HealthCare and UMR: in network

• HealthNet/AmBetter: Not in network

• Humana: Not in network.

• AHCCCS (Medicaid): We do not bill AHCCCS. AHCCCS members are welcome to see us as self-pay patients. This included United HealthCare Community plans.

• Medicare: We do not accept Medicare of any kind. This includes supplemental plans such as United Healthcare/Medicare, Medicare Advantage. In almost all cases, you will need to see a Medicare-network provider if you are age 65 or older.

• Oscar: Oscar insurance has informed us that they will not be accepting our practice into their network. They declined to state a reason.
