  • Click here to schedule your appointment in real time.
     

  • You will receive new patient forms by email. Be sure to sign each one, and click "Finish and "Submit." (not "Save.")

  • In many cases, the patient portal cannot be accessed using a smart phone. Please use a desktop, notebook, or tablet.

  • For our patients under age 18, please ensure that you enter the appropriate name for a responsible party (a parent or guardian.)
     

  • If you do not see an appointment time or date that suits you, call our office: 520-300-6131.
     

  • Please allow about 60 minutes to receive a confirmation email.

 

  • Use your mouse, iPad/tablet screen, or trackpad to sign your name on each document.
     

  • Click Finish and Submit after completing each document (not "Save.")
     

  • If you need to send us a photo or copies of your insurance card, email these to reception@mensmedical.health.
